Care

Community Health Care is the foundation of Masithandane and we breathe CARE in everything we do. We provide home care services to the sick, elderly and vulnerable; health talks and screenings. Our Care and Respite centre (HUG) is open 24/7 for those needing short term care. 


We build partnerships an
d work collaboratively with the public and private sector as well as local organisations in order to serve those: living in Smutsville, Sizamile, Fairview, Karatara, Elandskraal, Barrington and Farleigh who have been infected/affected by: HIV/AIDS and TB as well as widows, orphans and the destitute.

The Masithandane Home and Community-based Care Programme offers care along a continuum following a life course approach for children, adolescents, adults & the elderly. The comprehensive care package includes primary health care and support, prevention of ill-health and the spread of the Coronavirus and health promotion activities including the taking of responsibility for your own care and that of your children.



With a team made up of a Professional Nurse, Counsellor and community health care workers, the programme provides access to person centred and integrated community quality health care using a holistic approach - physical, mental, emotional, spiritual.


It includes:        

                                                                                 

  • Screening/assessments of household members, living conditions, lifestyles and health in order to identify health needs and necessary early interventions
  • Drawing up and implementation of individual care plans , including home-based care for the severely health challenged for the client basedon referral documents and additional identified needs
  • Providing inter-mediate care
  • Teaching and motivating the patient and family members where possible to render the necessary care themselves
  • Promoting awareness of chronic conditions, encouraging healthy lifestyles and promoting adherence to chronic illness regimes including HIV, TB, Asthma, Hypertension, Diabetes Mellitus and Epilepsy thereby promoting self-care 
  • Protection and the prevention of injuries and violence
  • Referral of household members with health needs to the Primary Health Care Facility/Clinic
  • Referral of client to additional services if required (for example: Home Affairs for ID document / birth certificate; SASSA for grant applications, Social Worker for social problems)
  • Assisting with nutritional support if required  
  • Assisting with arrangements for confinement for pregnant women, as well as informing them where emergency help can be accessed         
  • Extending an invitation to join an appropriate support group
  • Offering health education and support for post-natal mothers and neonates; clients with chronic diseases; vulnerable children and adolescents
  • Support of patient family in the end of life care phase
  • Fostering healthy working relationships with the Primary Healthcare Facilities as well as available medical and allied health workers (medical practitioners, physiotherapists, dentists, optometrists, social workers, occupational therapists, dieticians) in order to access assistance when needed
  • Establishing networks to add value to awareness and educational campaigns
  • Communicating and managing information accurately, equitably and efficiently and monitoring and evaluation of the programme
